Incident reports

How to Handle Incidents

Check back regularly for updates.

No Fear Cafe Incident Report Form

https://www.cognitoforms.com/NoFearCafe/nofearcafesteamincidentreport

  • In the event of an incident/injury:
  • Immediately contact the site program director
  • The adult staff member who observed the incident should write a detailed description of what occurred.
  • Be sure to explicitly note if first aid was given, if there were weapons (and what type), or if the matter involved sexual misconduct.
  • Description of the incident should be given directly to the DPSCD program director.
  • The DPSCD program director will complete the final Qualtrics reports for the district and discuss the incident with the child’s family.
  • The more we report the better, it doesn’t matter how minor it is.

Shift Coverage & Absence Policy

Consistent staffing is vital for a smooth and successful experience for our students during the summer program. Your cooperation in coordinating shift coverage helps ensure everything runs seamlessly.

Notifying Management of Absence

  • Scheduled Appointments: If you need time off for a planned appointment (medical, personal, etc.), you must notify management at least 7 days in advance of your scheduled shift.
  • Emergency Situations: In the event of an unexpected emergency, you must notify management at least 24 hours prior to your shift start time.

How to Notify:

Contact Sarah DeVoe, Program Manager, via:

  • Cell: (248) 820-2927
  • Email: nofearcafe@gmail.com

⚠️ Note for Voicemails: If you are leaving a voicemail, you must include your full name, phone number, the date and time of your call, and the specific reason for your absence.

Emergency Absences

  • Definition: An "emergency" notice is considered anything shorter than 24 hours before your shift start time.
  • No-Shows: Failure to call or email to report an absence will be documented as a "no-show."

Thank you

We appreciate your understanding and cooperation in adhering to these guidelines.
